So what’s the process?

  • Our staff will discuss your needs with you and what services/assistance you may require. You will be asked to complete an initial checklist to ensure we are developing a personalized care plan that best suits your needs.

Once the initial care plan is developed, you will be given the opportunity to review and make any changes and consult our staff.

You will also include your short and long-term goals and your desired outcomes. and expectations from OneNess Disability Services.

  • Once you are happy with your initial care plan, we will ask you to sign a service agreement and mandatory consent forms so that we can commence delivering the care that you need. The service agreement will highlight your goals and list of services that you have chosen.

  • One of our staff members will get in touch with you on a regular basis to ensure we are helping you to meet your goals and that you are achieving your desired outcomes.

Your care plan and service agreement will be reviewed as per the terms decided during Discussing and Planning.
